NIOSH Spirometry Training: Does Your Technician Need It?

What is NIOSH spirometry certification?

NIOSH spirometry certification is a standardized training course — typically 16 hours over two days — that qualifies a technician to perform pulmonary function testing to the technical standard used in federal occupational health surveillance programs. The course covers instrument calibration, patient coaching, the American Thoracic Society (ATS) acceptability and repeatability criteria, and common sources of testing error. A technician who completes it and passes the practical exam receives a certificate valid for five years. The certification itself is not an OSHA mandate for every industry, but several federal standards point directly to it, and most third-party accreditation bodies treat it as the baseline qualification for anyone administering surveillance spirometry.

Which OSHA standards actually require it?

Coverage depends on which exposure triggered the testing. The clearest requirement sits in the respirable crystalline silica standard, 29 CFR 1910.1053, which requires that spirometry be performed by a technician with a current certificate from a NIOSH-approved spirometry training course, or by a physician or other licensed health care professional. OSHA's cotton dust standard (1910.1043) and the respiratory protection standard's medical evaluation provisions (1910.134) reference spirometry testing but do not name NIOSH certification as explicitly. Many employers apply the silica standard's technician requirement across all their occupational spirometry programs anyway — partly because it's the clearest bar OSHA has set, and partly because untrained testers produce data that won't survive a medical review or an OSHA records request.

What happens if an uncertified technician runs the test?

The test still produces numbers, but the numbers may not mean anything. Spirometry is effort-dependent — a rushed or poorly coached maneuver can look acceptable on the screen while badly understating a worker's actual lung function, or a subtle coaching artifact can produce a false decline that triggers unnecessary medical follow-up. A reviewing physician grading uncertified technician data typically flags a higher rate of unacceptable curves, which means retesting, delayed clearance decisions, and — in a silica-exposed workforce — a compliance gap if OSHA asks who performed the test and what training they hold. The certificate itself doesn't need to be produced for every test, but it needs to exist and be current when asked for.

How long does certification last, and what's the renewal process?

NIOSH spirometry certificates are valid for five years from the date of the initial course or the most recent refresher. Renewal is a shorter refresher course — typically 4 to 8 hours — that re-covers calibration, coaching, and grading criteria and includes an updated practical evaluation. Employers running an ongoing surveillance program should track certificate expiration the same way they track audiometric calibration due dates: on a calendar, not from memory. A lapsed certificate doesn't retroactively invalidate prior tests, but it does mean the technician isn't qualified to test until they renew.

Who should get certified: in-house staff or a testing vendor?

The math usually comes down to test volume. A facility running a handful of silica-exposed workers through annual spirometry may not justify sending a staff member to a two-day course plus every-five-year renewal, especially once instrument calibration and quality-control overhead are factored in. A facility with a larger surveillance population, or one running spirometry alongside audiometric testing and fit testing on the same day, often gets more value from training an in-house occupational health coordinator. There's no OSHA requirement either way — the standard cares about who performs the test, not who employs them.
