Combining Audiometry, Spirometry, and Fit Testing in One Onsite Visit

Most employers running hearing conservation, respiratory protection, and pulmonary monitoring programs end up scheduling three separate testing events, because the three programs come from different OSHA standards with different technicians and different equipment. Combining audiometry, spirometry, and fit testing into a single onsite visit is possible and common, but each modality has its own environmental and procedural requirements that don't automatically align just because the same employees are walking through the same door.

Why do employers want to combine these tests into one visit?

Every additional testing day means additional lost production time, additional scheduling coordination with supervisors, and additional disruption to shift coverage. An employer running annual audiograms under a hearing conservation program, periodic spirometry for a respiratory protection or surveillance requirement, and fit testing for respirator users typically has significant overlap in which employees need which test, since noise-exposed and respirator-wearing populations often intersect on a plant floor. One combined visit means one shift disruption instead of three, one appointment employees have to remember, and one vendor coordinating instead of three separate scheduling calls.

What does each test actually require that the others don't?

Audiometric testing under 29 CFR 1910.95 requires a quiet test environment meeting ANSI S3.1 ambient noise limits, typically a certified sound booth, plus a calibrated audiometer and a technician who can recognize an invalid threshold. Spirometry testing requires a calibrated spirometer, a technician trained to coach proper effort, and enough time per employee to get acceptable and repeatable curves under ATS criteria, since a rushed test produces unusable data. Fit testing, whether qualitative or quantitative, requires the specific respirator model the employee will actually wear, a fit test protocol appropriate to that respirator class, and enough space and time to complete the exercises without an employee feeling rushed through donning and adjustment. How should the visit be sequenced to avoid one test degrading another?

The order matters more than employers often assume. Fit testing exercises involve physical movement, talking, and sometimes mild exertion, all of which can leave an employee slightly winded, which is the wrong state to walk straight into a spirometry test that depends on a full forced exhalation effort. Audiometric testing should generally happen before fit testing rather than after, since donning and removing a respirator repeatedly during fit test exercises can leave residual pressure or warmth around the ear that some employees find distracting during a quiet listening task. A practical sequence many onsite programs use is audiometry first while employees are fresh, spirometry second in a calm state, and fit testing last, since it's the most physically active of the three and doesn't depend on quiet or the employee being at rest afterward.

What data actually needs to be shared across the three tests?

Employers sometimes assume combining the visit means combining the compliance records, and this is a real record with real consequences to keep separate. Audiometric records document standard threshold shift determinations under the hearing conservation program. Spirometry records feed respiratory surveillance and, depending on the exposure, specific standard requirements. Fit test records document the specific respirator model and fit factor for that individual, valid only for that model until the next required interval. A combined visit is an operational efficiency, not a combined record. Each result needs to land in its own compliant file structure even when all three happen on the same afternoon.

What logistics actually break when employers try to combine these without planning?

The most common failure isn't a technical one, it's underestimating total time per employee and running out of appointment slots.
